- If you have lost or misplaced your DIS Book, immediately report the loss by submitting the DIS Issue Form. Visit your nearest branch with your original Aadhaar ID proof. The branch will stamp your request with the date and time and verify your signature from the back office. If the signature matches, a new DIS book will be issued.
- If an authorised person is collecting the DIS Book on your behalf, they must submit an authority letter and self-attested ID proof and carry the original ID for verification. Their signature must match across the documents and registers. - In dormant accounts, DIS will only be issued to the client in person. In the case of joint accounts, any one holder can collect the DIS with all holders’ signatures on the request. For non-individual accounts, the request must be signed and stamped by the authorised signatory.
- If the signature does not match, the request will be rejected.
